Public Assistance Forms/Checklists for Applicants

Hundreds of cities and towns have applied for Public Assistance funds to help pay for public infrastructure repairs following last year's storms.  This process is new for many communities.
The forms below are intended to help Public Assistance applicants with closing out a project, requesting an extension, requesting a change in scope of work, and requesting alternate & improved projects.
